Release of Liability Form

All guests, including children & pets, must complete a Release of Liability form before entering 5T Farm.

• One form must be completed for each participant (parents/guardians sign for minors).

• You may fill out the form electronically and email it back , text a photo of the signed form, or bring it with you.
