Release of Liability Form

Every guest, including children of all ages, must have a completed Release of Liability form before participating in farm activities.

 

• One form must be completed for each participant. Parents or legal guardians must sign for minors.

 

• You may complete the form electronically and email it back, text a photo of the signed form, or bring it with you.

 

• Pets are not permitted unless 5T Farm has approved an animal in advance for a specific photography session. Approved animals require a separate Pet Release Form.
